Identify your business; You can do this by putting the letterhead on the top of the application.

Standard information; Full Name, Address, Phone numbers (all), Social Security number, Education acquired, Graduation dates, Extra education received in other areas, Military experience, past jobs, contacts at past jobs are all needed.
I stay away from sex, race,and date of birth. Avoid any type of discrimination. You can usually settle those things at a sit down interview. I also stay away from religion, unless it is a religion related job.
Also have a phrase that a back round check will be done and that maybe a drug test will be needed. Then ask the question, “Have you been arrested for any reason? ” Always ask the question second so that they read the phrase first.
References; You actually have both types on this application. You have past job contacts already. Now, you ask for character references. Avoid family members. Be sure to get addresses and phone numbers.
If an individual has a handicap, ask what type of assistance you can provide to help them with the position they are applying for. You want to be as helpful as you can without discrimination against a handicapped individual.
Lastly, Ask specific questions that are related to an individuals experience for the job you have in mind. They won’t be like the earlier questions.
